Medical Upload Portal

Medical documentation and forms should be submitted directly to Medical Review Services by the treating physician. Documents uploaded by the customer may not be accepted or processed. Receiving information directly from the physician helps ensure the documentation is accurate, complete, and reviewed as quickly as possible.

When uploading documentation, please include the customer’s email address. This allows Medical Review Services to send the customer helpful updates—such as confirmation when documents are received, when they have been processed, or if more information is needed—so they can stay informed throughout the review process.
